Evidence of Identity

The main and joint applicant (e.g. husband, wife or partner where appropriate) must provide evidence of identity by providing either:

1 document from list A or
2 documents, 1 document from list B and 1 from list E or
3 documents, 1 document from list C and 1 from list D and 1 from list E

List A Full current name, address & national Insurance number must show	DWP benefit award letter (State pension, Pension Credit, Attendance Allowance, Disability Living Allowance etc.) dated within the last 12 months. HMRC Tax credit or Child Benefit award letter dated within the last 12 months. Computer-generated payslip showing the employers name, dated within the last 3 months Bank Statement dated within the last 3 months showing a DWP benefit payment (State pension, Attendance Allowance, Disability Living Allowance etc.). Bank statement dated within the last 3 months showing a HMRC Tax credit or Child Benefit payment.
List B Full current name & address must show	Council tax reduction award letter for the current year Housing benefit award letter for the current year Bank, Building Society, Post Office or Credit union full monthly statement dated within the last 3 months Mortgage statement from a recognised UK lender dated within the last 12 months
List C Full current name must show	Birth certificate (short or full) Adoption certificate Deed-poll document Marriage/Civil Partnership certificate Decree of Nullity for marriage and civil partnerships Current signed passport Residents permit issued by the Home Office to EEA nationals National Identity card bearing a photograph of the individual Naturalisation Certificate Current firearm or shotgun certificate Current DBS (Disclosure & Barring Service) papers Current UK or EES photo card driving licence Current full old paper driving licence
List D Full current address must show	Utility Bill (gas, electric, landline phone bill) dated within the last 6 months Current TV licence Satellite, Broadband provider bill dated within the last 6 months LA council tax bill for the current year Rent agreement or rent statement dated within the last 12 months Contents or building insurance policy dated within the last 12 months Current notice to vacate property
List E Full current name & national insurance number must show	National Insurance number plastic card or paper equivalent HMRC self-assessment letter or tax demand Old computer-generated payslip P60 P45 Old bank statement Old DPW or HMRC letter HMRC construction industry registration card (CIS4, CIS5 or CIS6)
